Lisa and Denise return from the vacation, wearing younger versions of Carol and Tom's bodies, excited from all the new experiences. Denise tries to switch back only to discover that she is now pregnant. Lisa won't leave Denise to go through pregnancy alone and so they ask Tom and Carol to take over their former lives for the next 9 months. Just until the baby is born.
At the end of the 9 months all four prefer their new bodies and lives and toss the medallion into the ocean.
Maybe they should stay as the new couple, whlie their daughter keeps their forms
